Who we are

This website, jasongoldmanmd.com, is operated by the medical practice of Jason M. Goldman, MD, MACP, in Coral Springs, Florida. This policy explains what information the website collects, how it is used, and the choices you have. It applies to the website only.

Information you share with the practice as a patient, in person, by phone, or through your medical records, is governed by the practice's Notice of Privacy Practices, which the office provides separately.

Information you give us

The only form on this website is the appointment request. It asks for your first and last name, your email address, your phone number, and the service you are interested in. We use this information to respond to your request and schedule your visit. Submitting a request does not make you a patient of the practice or create a medical record.

The request is delivered to the office by email through an email delivery service that processes it on our behalf. The form does not ask for medical information and should not be used to send it; please share medical details with the office by phone or at your visit. If you give us an email address, the office may reply to you by email.

The patient forms available on the contact page are downloaded to your device and completed offline. The website does not receive them.

Information collected automatically

Like most websites, this site records standard technical information when you visit, such as your IP address, browser and device type, the pages you view, and the date and time of your visit. Our hosting provider may record this information in its server logs.

We use Google Analytics to understand how visitors use the site, such as which pages are viewed and from what kind of device. Google Analytics uses cookies and similar technologies and may collect information such as approximate location and browsing behavior on this site. We have chosen not to load Google Analytics on the appointment request page. You can learn how Google uses data at policies.google.com/technologies/partner-sites and opt out with the Google Analytics opt-out browser add-on.

Cookies

This website does not set cookies of its own. Cookies may be set by the third-party services described in this policy. You can block or delete cookies in your browser settings. The website's own pages and the appointment request form do not depend on cookies, although some embedded content from other providers may not display.

This website does not respond to browser "Do Not Track" signals.

Third-party content

Some parts of the site are provided by other companies, which may collect information according to their own privacy policies:

  • Reviews shown on the home page are displayed through a third-party widget that pulls public reviews from the practice's Google Business Profile. It loads only when you scroll to that section, and the widget provider and Google may set their own cookies, including cookies from Google advertising domains.
  • A video on the home page is embedded from YouTube in privacy-enhanced mode and loads only when you press play.
  • A map on the contact page is provided by Google Maps and loads only when you choose to show it.
  • Links to social networks, review sites, and the credential verification page take you to those services.

How we share information

The information you submit through the form is used only by the office and the providers that deliver it to us, our hosting and email delivery providers; we do not sell it or use it for advertising. Technical and usage information is collected by the hosting, analytics and embedded services described above under their own policies. We may also disclose information when required by law or to protect the rights and safety of the practice and its patients.

How long we keep information

Appointment requests are kept for as long as needed to respond to you and schedule your visit, and for any longer period required by law or the practice's record-keeping policies. Analytics data is retained according to the settings of our Google Analytics account.

Security

The website is served over an encrypted connection (HTTPS), and we take reasonable steps to protect the information you send us. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.

Children

This website is not directed to children, and we do not knowingly collect personal information from anyone under 13. The practice sees patients aged 16 and older.

Your choices and rights

You can contact the office to ask what information we hold from your website submissions, to correct it, or to request that it be deleted. Depending on where you live, you may have additional privacy rights under applicable law; you can exercise them by contacting the office using the details below. You can also disable cookies in your browser and opt out of Google Analytics as described above.

Changes to this policy

We may update this policy from time to time. If we make a material change, we will post the updated policy on this page with a new effective date.
